Health Services
Our school nurses provide an essential resource to our students, families, and staff. What do they do each day? The list is long, but here are a few highlights:
Assess student health symptoms and determine if a student should remain at school, go home, and/or be seen by a physician.
Manage students' medication.
Provide health screenings and referrals to family health care providers.
Track student immunization requirements and coordinate other state health requirements.
Educate students on disease prevention and other general health topics.
Required Health Screenings
Physical - all Kindergarten students (not including sports physical for MS/HS students)
Meningoccal vaccine - BY LAW, students entering 7th and 12th grade MUST have received this vaccine to attend school and participate in any school-related activities. The state has not provided any exceptions for this rule.
Students entering 7th grade (if born after September 15, 2004) – 1 dose of meningococcal vaccine received on or after age of 10 – a booster dose will be needed at age 16.
Students entering 12th grade (if born after September 15, 1999) – 1 dose of meningococcal vaccine on or after the age of 16.
Your doctor will provide you with a printed form.
Dental screening - all Kindergarten and students new to the district. All 9th grade students.
Immunization records - all Kindergarten and students new to the district (must be up-to-date before attending school)
Your doctor will provide you with a printed form
TDaP immunization - All 7th grade students
Your doctor will provide you with a printed form
Vision screening - all Kindergarten and 3rd grade students
Immunization Requirements
Immunization Requirements Iowa Code 139A.8 requires all students enrolling in an Iowa school to submit proof of immunizations to the admitting official using an Iowa Department of Health and Human Services Certificate of Immunization. Documentation of a valid Certificate of Immunization Exemption (Religious or Medical) or Provisional Certificate of Immunization may be submitted in lieu of a Certificate of Immunization. Requirements for valid exemptions may be found in 139A.8 and Iowa Administrative Code 641-7. Refer to House File 299 regarding immunization exemption communication requirements. Immunization Certificates The law authorizes medical and religious exemptions to immunizations and provides for provisional enrollment of a child for up to 60 calendar days if the child has begun the required immunizations. Iowa Code § 139A.8(4), (5); 641 IAC 7.3, 7.7.
