Transition Alliance Program (TAP) was created in partnership between the Division of Vocational Rehabilitation Services and Clarke Community Schools. Youth participating in TAP are provided opportunities to develop or enhance skills in academic, vocational and independent living competencies. TAP addresses the
individualized needs of youth to continue as lifelong learners and become successful, productive and contributing citizens.
Effective collaboration between TAP, schools and community resources helps youth establish relationships and build effective teams. This allows TAP participants to become aware of opportunities within their community and develop additional skills that benefit them for life.
This Community Resource Map (CRM) website is sponsored by the Transition Alliance Program. TAP and the Community Resource Mapping Committee invite you to explore the following information and links on this website to research and develop an individualized transition map for your future.
